At All Heart Home Care, our commitment to making exceptional in-home care accessible aligns perfectly with the goals of CalAIM. By partnering with this initiative, we work to level the playing field for non-medical care across the state. See below how our collaboration with CalAIM helps us better serve our clients.

Expanding Service Options

Personal Care and Homemaker Services

We offer tailored support to help individuals maintain a healthy and independent life at home. Our services include light housekeeping, meal preparation, medication reminders, companionship, and assistance with daily activities. By keeping our clients in their homes, we promote stability and comfort while preventing the need for costlier institutional care.

Benefits: Free Personal Care and Homemaker Services are provided to qualified members.

Who Qualiﬁes: Molina Medi-Cal managed members who meet one of the following criteria:

Member is in the process of obtaining IHSS
Member has IHSS and is need of more hours

How to identify candidates: Any member meeting the qualification criteria is eligible and can be referred to All Heart Home Care. There’s is no cost for Personal Care and Homemaker Services.

Respite Services

Recognizing that primary caregivers sometimes need a break; we provide respite care that temporarily relieves them of their duties. This ensures that the primary caregiver can recharge, secure in the knowledge that their loved one is receiving quality, compassionate care.

Benefits: Up to 336 hours of free Personal Care and Homemaker Services each calendar year.

Who Qualiﬁes: Molina Medi-Cal managed members where their caregivers need respite to prevent burnout.

How to identify candidates: Caregivers of members needing relief. The member does not need to have IHSS.

Streamlined Care Coordination

CalAIM enables a more coordinated approach to care by assigning eligible Medi-Cal recipients an Enhanced Care Manager (ECM). These professionals work directly with us at All Heart Home Care to assess each client’s unique needs, create personalized care plans, and coordinate services seamlessly. Whether a client is beginning the process to qualify for In-Home Supportive Services (IHSS) or needs a reassessment of their current hours, our team collaborates closely with the ECM and the client’s support network.

Empowering Clients and Caregivers

One of the most innovative aspects of our CalAIM partnership is our role in connecting clients with trusted caregivers from their own network. When a client is eligible for CalAIM-supported services, All Heart Home Care helps identify someone within their circle who can provide care. We then become the employer of record for that caregiver, handling essential functions such as training, payroll, and ongoing support. This model not only offers personalized care but also strengthens the bonds within families and communities.

CalAIM in Action: A Community Example

Consider the story of Maria, a long-time resident of our community who has always cherished her independence. After facing health challenges that made daily activities more difficult, Maria found herself at risk of hospitalization. Through the CalAIM program, Maria’s Enhanced Care Manager connected her with All Heart Home Care. We quickly worked with her family to identify a caring relative who could serve as her in-home caregiver. With comprehensive training and ongoing support from our team, Maria now receives personalized care at home, allowing her to continue pursuing her hobbies and social activities with confidence—all at no out-of-pocket cost to her.
